Participant Directed Managed Services

We offer home and community-based services to people who experience developmental disabilities to help them stay as independent as possible. When it comes to managing those services, everyone has different needs. Choosing the Consumer Directed option allows individuals more control over their program services if they are able to manage them responsibly. Gateways Adult Day Center is designed to help meet the social, cognitive, physical, and medical needs of chronically ill and disabled adults 18 years and older in a caring and friendly setting. We offer socialization, emotional support, physical activity, nutritious meals, cognitive exercises, medication administration, and medical oversight. In addition to meeting the needs of our participants, we also strive to support caregivers by giving them free time for themselves and staff to speak to about their loved ones. We are typically open from 9 am to 3 pm, Monday through Friday year-round.  We are part of a special partnership located on the Alvirne High School Campus located at 200 Derry Street in Hudson. Adult Service Coordination

We provide person-centered planning and support to individuals who are determined eligible to receive services. Service Coordinators work with individuals and teams to develop service plans in partnership with vendors to meet shared, desired outcomes.

Young Elderly Help

Choices for Independence (CFI)

CFI offers a wide range of services to individuals 18 years and older, as determined by the Department of Health and Human Services and the Bureau of Elderly and Adult Services.
We work with participants to optimize their independence at home and within the community through a person-centered approach.

*Gateways CFI Case Management provides conflict-free case management and is unable to refer CFI clients to Gateways for Seniors' home care service program.
