Consumer Directed Services
We offer home and community-based services to people who experience developmental disabilities to help them stay as independent as possible. When it comes to managing those services, everyone has different needs. Choosing the Consumer Directed option allows individuals more control over their program services if they are able to manage them responsibility. Our team will help to decide what services are needed and how to maximize usage of Medicaid funds.
Choices for Independence (CFI)
CFI offers a wide range of services to individuals 18 years and older, as determined by the Department of Health and Human Services and the Bureau of Elderly and Adult Services.
We work with participants to optimize their independence at home and within the community through a person-centered approach.
*Gateways CFI Case Management provides conflict-free case management and is unable to refer CFI clients to Gateways for Seniors home care service program.

We offer case management support to individuals with intellectual disabilities and brain acquired injuries. We advocate and oversee supports and services to ensure continuity of care.
