Participant Directed Managed Services

We offer home and community-based services to people who experience developmental disabilities to help them stay as independent as possible. When it comes to managing those services, everyone has different needs. Choosing the Consumer Directed option allows individuals more control over their program services if they are able to manage them responsibly. Our team will help to decide what services are needed and how to maximize usage of Medicaid funds. Please call (603) 882-6333 to schedule an intake appointment.

Gateways Adult Day Center is designed to help meet the social, cognitive, physical, and medical needs of chronically ill and disabled adults 18 years and older in a caring and friendly setting. We offer socialization, emotional support, physical activity, nutritious meals, cognitive exercises, medication administration, and medical oversight. In addition to meeting the needs of our participants, we also strive to support caregivers by giving them free time for themselves and staff to speak to about their loved ones. We are typically open from 9 am to 3 pm, Monday through Friday year-round.  We are part of a special partnership located on the Alvirne High School Campus located at 200 Derry Street in Hudson. Call for more information: (603) 883-0994.
