Early Supports & Services Program empowers families to choose supports that will meet the developmental challenges of their child, respecting the family's values, needs, culture and priorities.
Families are educated and prepared, through a team approach to assist a transition into appropriate community programs, ensuring the uninterrupted developmental growth of each child. |
Early Supports & Services Program was developed in collaboration with area providers to help families access Family Centered Early Supports and Services, and explore choices for services and other needed supports.
Early Supports & Services Program is available to children birth to three years of age who have an established condition, who have a 33% delay in one or more developmental areas, or who at risk for a substantial delay.
Families will participate in a family-centered evaluation and assessment by a multidisciplinary team. Eligible families will receive Early Supports & Services as agreed upon in their Individualized Family Support Plan.
