As a progressive team of human services professionals our case managers may provide assistance to individuals who have developmental disabilities through service coordination, advocacy, and quality assurance services or provide all necessary supports to facilitate participants to plan, implement, monitor, and adjust budget service hours and/or financial allocation to fully utilize account resources. This position would required a minimum of a bachelor's degree or high school diploma with one or more years of college level education; and 2-3 years direct experience in planning and facilitating groups or prior case work or related experience with people who have developmental disabilities and their families. Personal Care Services is a non-medical, hands-on service to Home and Community Based Care-Elderly and Chronically Ill individuals.
A Personal Care Services worker may provide support at home or in the community with:

- Walking / Mobility

Speech & Language Pathologists/Occupational Therapists
Join our transdiscplinary team! Family Centered Early Supports & Services are resources that are provided to children from birth to three years of age and their families. This includes a developmental evaluation, therapeutic interventions, guidance and instruction, as well as emotional support.
